Search for duplicates is missing hits (probably when line numbers are shifted). See for example bug 154895 and bug 155712.

Rob: perhaps we should add a simple fuzz factor for line numbers, e.g. +/- 10?

When you do this add a simple test that asserts matches with a the 4 interesting entry offsets (+11, +10, exact, -10, -11).  I wonder if 10 is enough for the fuzz factor though, because it won't capture a moved or deleted method.  Why don't you do 20 for now and we'll see how many false positives we get.  A better way to do this would be to verify that it is the same exception and the same top stack frame.
