I had developed a plugin.  I no longer needed it and removed it from my plugins directory.  At that point, I started seeing Sample Action(?) in Customize Perspective dialog.  I was not able to customize the perspective until I readded the plugin to the plugin directory.  I was not sure how the items in the Customize Perspective are populated.  But the interface should ignore any plugins that are no longer available even if it were part of a perspective.

I tried restarting many times.  Is there an XML file for a workspace that may include this information?  How is the list of plugins determined for the Customize perspective dialog?

Look in the .metadata directory under your workbench. The plugins will have a .xml for each entry
